Whether you need to request a grade appeal or register a visitor, here you can find all relevant forms for registration, records, and academic scheduling at 51风流.

    Records forms

    • : Please allow up to three weeks for departmental approvals and processing.聽
    • :聽The fastest way to change your address is online via Workday. If you cannot access Workday, please use this form.
    • :聽Legal name and gender changes can be made, with appropriate supporting documentation, in Workday. If you cannot access Workday, please use this form.

    • :聽A student may design their own major with approval of this form. Must be submitted no sooner than the first term of the sophomore year and no later than the first term of the senior year.
    • :聽Students may appeal the grade assigned to them in a course. Submission of an appeal does not guarantee a grade change. See the complete grade appeal policy in the Bulletin ().
    • Leave of Absence Request:聽All Leave of Absence requests are now in聽. Go to the Academics Application and look under General Help.
    • :聽Students unable to finish a course due to special circumstances may request an incomplete grade from their instructor.聽
    • :聽Request an exception to academic policy by submitting a petition to the Undergraduate Curriculum Committee. Submission of a petition does not guarantee approval.
    • :聽Request to add, drop, or withdraw from classes by submitting this form. Submission of a petition does not guarantee approval.

    • Additional License Advanced Practicum Registration, pre-2015 (PDF): For students who were admitted to the additional license program prior to Fall 2015.
    • Additional License Advanced Practicum Registration (PDF): For students who were admitted to the additional license program in Fall 2015 or later.
    • :聽Undergraduate students may earn a maximum of 4 credits toward graduation by assisting faculty with their teaching responsibilities. Apprentice teaching is graded on the Pass/No Pass scale.
    • :聽Register for a course on a no-credit, ungraded basis for a flat fee. This form is also for visiting Amity Scholars.
    • :聽Register for a course on a no-credit, ungraded basis for a flat fee.
    • :聽Collaborative research projects offer undergraduate students the chance to pursue independent study in greater depth with a faculty member, often in conjunction with the professor鈥檚 own research.
    • :聽Complete this form to register for a course outside of your school or program.
    • :聽Students with a minimum 3.25 GPA may submit proposals by May 1st of the spring term of their junior year.
    • : Undergraduate students use this form to apply to add the LEAP designation to one of the following types of experiences: Apprentice Teaching, Collaborative Research, or Independent Study.聽This form is not to be used with internships.
    • : Apply to pursue an area of study not covered by the established curriculum.
